I think the very last line ("the tears in my ey-y-yes" in Chorus 3) is actually D -> F -> G and then back up to A until the end of the song.

V. COOL, I really love this song. This has a nice layout, the chords are written above in the right places; and I also like the way the varius chords are written out at the bottom. Nice one!
